What we're solving

Changes close faster than the documents can follow.

Every change is approved, built, and closed, and the drawing gets revised. The procedure, the cause-and-effect sheet, and the HAZOP worksheet follow when someone has the time, and with dozens of changes a year some never do. The revalidation opens on documents that describe the plant at three different dates.
